3.3 The Cumulative Casualty Record

4. The Fukushima Daiichi Cascade Failure
4.1 The Initial Cascade (11–14 March)
The governing fact is a design-basis failure: Fukushima Daiichi's coastal defences were rated to a 5.7-metre tsunami, and the wave that struck at 15:27 JST on 11 March — 41 minutes after the earthquake — inundated the site to 14-15 metres. Of the plant's six boiling-water reactors, Units 1-3 were operating (Units 4-6 were in scheduled maintenance, fuel stored in the spent-fuel pools); the 14:46 earthquake had already cut off-site power and triggered automatic scram, with the emergency diesel generators activating as designed. The tsunami's exceedance of the seawall flooded those generators, causing a total loss of AC power — a Station Blackout — leaving only battery reserves; it was this loss of power, not the earthquake, that made the meltdown cascade unavoidable.
The cascade unfolded over the next four days: Unit 1's cooling failed by 18:18 on 11 March, with the core uncovered and meltdown underway within hours; hydrogen explosions breached containment at Unit 1 (15:36, 12 March), Unit 3 (11:01, 14 March), and Unit 4 (06:14, 15 March, via hydrogen migrated from Unit 3 despite Unit 4 being offline for maintenance). Radioactive release continued through 15-16 March as the evacuation perimeter expanded to a 30-kilometre zone.
4.2 The Operational Crisis Management
The operational crisis-management through 11-31 March was conducted by TEPCO under the post-Station-Blackout framework. 6. The Nuclear-Policy Trajectory
6.1 The Kan Government's July 2011 Phase-Out Decision
Prime Minister Kan Naoto announced on 13 July 2011 the Genpatsu Tachidanen Sengen (Declaration on Departing from Nuclear Power) committing Japan to a long-arc-trajectory of phasing-out nuclear power. The decision was the most-substantial post-3.11 policy-modification at the time and was a substantial-departure from the pre-2011 LDP-and-DPJ-established nuclear-energy-policy framework. - The extensive-failures position (the NAIIC Report's selected criticisms; selected-LDP-aligned commentary; selected academic-critical commentary): the Kan government's crisis-period governance contained broad-failures including: the SPEEDI-radiation-projection-data dissemination delays; the 12 March helicopter-tour-of-Fukushima political-distraction; the post-15 March cabinet-deliberation-instability; the broader institutional-political-coalition-management. The cumulative failures contributed to suboptimal cumulative disaster-outcomes.
- The mixed-assessment position (Hatamura Report; IAEA report; broader academic commentary): both major-engagement and selected sweeping-failures were operative; the eventual academic-historical assessment requires engagement with both elements.
8.2 The Post-3.11 Nuclear-Policy Reversal
The post-2013 selective-restart programme's reversal of the 2011-Kan zero-nuclear commitment has been the subject of substantial post-event commentary. Three positions:
- Appropriately-cautious: the post-2013 NRA-administered selective-restart under the New Regulatory Standards represents appropriate-caution; the cumulative energy-policy environment requires nuclear-power as part of the cumulative decarbonisation architecture.
- Premature-restart: the post-2013 selective-restart represents premature-reversal of the 2011-zero-nuclear commitment; the cumulative post-3.11 lessons have not been adequately-integrated into the post-2013 framework; the broader renewable-energy-architecture should be the principal replacement-framework.
- Partially-appropriate-with-policy-design-defects: the cumulative restart-trajectory contains selected appropriate-elements (the NRA-independence; the New Regulatory Standards) but selected policy-design-defects (the post-GX cumulative acceleration; the cumulative operational-life-extension to 70 years).
8.3 The Cumulative Public-Health Outcomes
The cumulative public-health outcomes of the Fukushima radiation-exposure have been substantially-monitored through the post-2011 Fukushima Health Survey conducted by Fukushima Medical University. The cumulative-2024 findings: approximately 270 cases of thyroid-cancer detected in the post-2011 surveillance population (substantially-elevated relative to pre-event baseline; the cumulative causation-attribution between cumulative radiation-exposure and surveillance-screening-effects has been the subject of substantial-academic debate). The cumulative radiation-exposure-related deaths through 2024 have been substantially-low; the cumulative disaster-related-deaths from the broader displacement-and-stress-related causes have been substantially-larger.
The cumulative academic-and-civil-society contestation on the public-health outcomes has been substantial; the eventual long-arc-public-health record will require continuing-decadal-monitoring.
9. Conclusion — The 3.11 Disaster as Foundational Antecedent and Continuing Reference
The 11 March 2011 Tohoku Earthquake, Tsunami, and Fukushima Daiichi Nuclear Disaster was the most-consequential single-natural-disaster-and-cascading-industrial-accident event in Japan's post-1945 history and the foundational-antecedent for the December 2012 LDP comeback (JP-B-01). The cumulative disaster-trajectory through 2011-2025 has produced: approximately 22,000 cumulative deaths-plus-missing in the Tohoku region; approximately 32 trillion yen of cumulative government reconstruction-expenditure; the substantial-modification of the post-2011 Japanese nuclear-energy-policy framework; the cumulative political-coalition-trajectory that conditioned the post-2012 Abe-2 government and the broader post-3.11 political-economic-environment.
The 3.11 disaster's foundational-significance for JP-B-01 (Abe's Return to Power) operates on three levels. First, the post-3.11 DPJ political-coalition credibility decline was the structural-political-trajectory that the December 2012 LDP comeback exploited. Second, the post-3.11 economic-policy-pressure environment (including the cumulative reconstruction-financing requirement, the cumulative yen-strengthening-pressure, the cumulative energy-import-cost increase) was the principal economic-policy-environment that the 2012 Abenomics framework was designed to address. Third, the post-3.11 institutional-and-policy-questions (including the post-2012 NRA-and-New-Regulatory-Standards framework; the cumulative reconstruction-architecture; the broader disaster-preparedness framework) have been the institutional-architecture against which the post-2012 governance has operated.
Three structural questions will determine the long-term verdict on the 3.11 disaster and on the post-2025 trajectory.
First, whether the cumulative-Fukushima-prefecture-reconstruction produces durable-population-restoration and broader community-recovery, or whether the cumulative displacement-and-demographic-decline trajectory will continue through the post-2025 period.
Second, whether the post-2013 NRA-administered nuclear-restart trajectory produces durable-safety-architecture or whether the post-2025 cumulative restart-acceleration produces selective-safety-architecture-erosion.
Third, whether the cumulative public-health-monitoring of the affected-population produces a durable-low-mortality-and-low-morbidity record or whether the cumulative decadal-trajectory reveals selected sustained-public-health-impact.
This document, written in the post-2025 era and approximately 14 years after the 11 March 2011 disaster, records the cumulative disaster-trajectory, the post-2011 reconstruction architecture, the post-3.11 nuclear-policy trajectory, and the contested-record as they have crystallised through mid-2025.
